TERMS & POLICIES AGREEMENT

I understand that if I am more than 10 minutes late, my appointment will be forfeited immediately, and the deposit I paid is non-refundable.

I understand that I can reschedule my appointment more than 48 hours prior to my appointment, or I can cancel my appointment at any time, and that the deposit fee is non-refundable. I understand there are no exceptions to this rule.
- ACNE: Acne harbors high levels of bacteria. I understand that Katrina can NOT tattoo over/around acne as a safety precaution to prevent serious infections such as staph & MRSA. It will be my responsibility as the client to thoroughly scan my eyebrows for acne in the days before and on the day of my appointment. If acne is present, I will send photos of the acne & eyebrows to Katrina for review and if needed, we will reschedule my appointment without penalty as long as I communicate this with Katrina before my appointment. I understand that if I show up to my appointment with acne in/around my eyebrows that I failed to communicate with Katrina, I will be sent home, the deposit I paid is non-refundable, & I will have to pay a new deposit in order to book a new appointment.

I understand that it is my responsibility to immediately visit browsbykatrina.com after booking my appointment to view and follow the pre-care instructions for best possible results.

If I have previous work tattooing/microblading/ombre brows done on my eyebrows by another artist, I have received written approval for my procedure with Katrina via email.

I understand that if I do not attend the required touchup appointments within the required touchup time frames (6-8 weeks refinement session from my initial microblading appointment, and every 6-18 months thereafter for maintenance), that I will be reset to a new client and will incur new client fees should I choose to book another appointment. I understand that there are zero exceptions to these rules.

I understand that all appointments are reserved with non-refundable deposit fees at the time of scheduling.

I understand that legally recognized service animals may accompany me to my appointment, and that emotional support animals and/or pets are NOT allowed to accompany me to my appointment.

I understand that I may bring a guest with me to my appointment but they are not allowed in the procedure area and must wait in the waiting room.

I am 18 years of age or older, and I understand that a valid ID is required to be presented to the artist to prove as such.

I understand that Brows By Katrina LLC does NOT perform Microblading on pregnant or breast feeding women, or any persons fighting life-threatening illness.

I understand that if I am sick or have any onset symptoms of an illness, that I should reschedule or cancel my appointment.

I understand that if I arrive for my appointment with symptoms of illness, Brows By Katrina LLC will refuse service.

I understand that I will be given after care instructions upon completion of my appointment(s) and that these instructions are also available online at browsbykatrina.com. I understand that it is my responsibility to follow these instructions exactly for best results.

I understand that results will vary per individual and that there are no guarantees as to how well the tattoo will take on my skin in particular.
